CWEA, MWEA and HWEA Certificate Holders

	Recertification & Renewal / Approved Organizations & Online Courses Certificate holders must renew their certificates annually and recertify by completing at least 12 contact hours of continuing education every two years.

	Contact Hours Worksheet (optional) This worksheet is optional. Do NOT submit worksheet without actual copies of your contact hours/certificate(s). You can use this worksheet to to record and organize your contact hours/certificate(s).
